Questionnaires

Questionnaires allow you to collect learner input, reflections, or feedback through written or voice responses, multiple choice questions, and rating scales. They’re available to add as units in any Pathway.

📍 Accessing Questionnaires

To add or create a questionnaire:

1. Navigate to the Pathways tab in Bodyswaps Go.

2. Open an existing Pathway or create a new one.

3. Inside the Pathway editor, click Add a Unit!

4. Select the Questionnaires tab at the top.

From here, you can:

• Add an existing questionnaire to the Pathway

• Or click Create Questionnaire (top right) to build a new one

✏️ Creating a Questionnaire

After selecting Create Questionnaire, you’ll configure the following Questionnaire details:

• Name your questionnaire

• Choose the visibility setting (Private, Organisation)

• Add a short description 

Once you select Create & add questions:

Add Questions

Choose from the following response types:

• Freeform – Learners can respond using text or voice-to-text input

• Multiple Choice – Single or multi-select options

• Likert Scale – Rating scale (e.g. 1 to 5) for opinions or satisfaction

🛠️ Use the tools provided to:

• 🗑️ Delete unwanted questions

• ⬆️⬇️ Reorder questions to structure your survey logically

Save and Assign

When you're done building your questionnaire:

• Click Save Questionnaire to store it for future use

• Click Add to Pathway to immediately include it in your current Pathway
